SLCS an agency servicing individuals in supported living services where emphasis is placed on patterning each service category to fit the needs as well as the wants for each individual served. We ensure that each pattern is unique, adaptable and not repeatable. Our method of providing services contrasts with programs that use a curriculum model that everyone moves through regardless of their limitations. SLCS value the choices, needs, and satisfaction of people with developmental disabilities in addition to time-limited, measurable, instructional and behavioral goals.

We Can Assist You:
We will help you live in your own home or apartment by yourself or with friends, help you to learn how to do all of the things that you need to know about taking care of yourself and your home and living independently in the community.
We will help you learn about money by assisting you with paying your bills, as well as assisting you with housekeeping skills, grocery shopping, cooking simple meals, transportation , being safe and more…….
- Provide some transportation to assist you with working on goals and getting around.
- Participate in your monthly, quarterly and 1:1 meetings with your support coordinator.
- Develop your Implementation Plan, housing survey, financial profile and help you with formulating new goals and objectives.
- Supervise and monitor your attainment of goals within your home, program and community.
- Daily document your progress for each contact.
- Act as a liaison between your providers, parents or programs through both written and verbal daily communication (progress notes turned in by the 9th of each month).
- Advocate for you while in the community.
- Responsibly supervise and schedule your medical appointments.
- Maintain and ensure your health and safety standards.
- Assist you with developing and planning your activities.
