- UCMB will provide Medicaid form’s our providers may need, such as; demographic changes (new address, bank account for direct deposit, ect.)
- Upon request, agent will provide eligibility check monthly for recipients.
We will provide any Medicaid forms a Provider may need to stay up to date with Medicaid policy and procedure. Examples: New business or home address, telephone number or new/change of bank account for direct deposit users, NPI and more. These changes must be recorded in the Medicaid system in a timely manner to ensure prompt payment for services provided.
